3. To Whose Attention
4. Description of Job or other details they want spelled out
5. Do they need to be listed as additional insured and or Loss Payee or Mortgagee
6. Fax Number or Email Address

Please Note that we will not mail originals to you and certificate holder is sent by other mode unless requested to do so.
